BREEDER / BUYER SALES CONTRACT

This agreement is made between Karen Firlotte of Sidekick North American Miniature Australian Shepherds, hereafter referred to as BREEDER and ____________________, referred to as BUYER, address below:

1. Description

Breeder has sold to buyer the puppy described below:

2. TERMS

a) As payment for the above described puppy, BUYER agrees to pay BREEDER the sum of __________

b) BUYER agrees to provide a veterinarian checkup within 48 hours after receiving the above named puppy. In the event that medical problems arise, as evidenced by a veterinarian's statement, BUYER agrees to return puppy within 24 - 48 hours to BREEDER FOR A FULL REFUND.

c) BUYER agrees to provide on going veterinary care for this puppy on into adulthood for as long as BUYER owns the animal. Further, BUYER agrees to provide adequate socialization, training and compassionate care to ensure for a well adjusted companion animal.

d) BUYER agrees that at any time after 7 days of receipt of payment to BREEDER, he, or she decides not to keep this animal, BREEDER will be notified first, and have take-back privileges over any other person. A replacement puppy will be offered at a future time, When the BREEDER has a litter of puppies.

e) BUYER agrees to pay for all shipping costs that may occur.

f) BUYER agrees that this animal will never be placed in a pound or rescue facility for any reason and that contact with the breeder will be ongoing.

3. WARRANTY

This puppy is warranted only for the purpose of being a companion animal. While it may be capable of performing other functions, no warranty is given as to its fitness for any special purpose.

BREEDER guarantees puppy is healthy and well socialized at time of sale, free of known contagious diseases and had vaccinations required for its age at the time of this sale.

BREEDER guarantees this puppy from hip or eye defects of a genetic origin for a period of 2 years. In the event hip or eye defects occur, BUYER will provide written certification from the Orthopedic Foundation for animals (OFA) or the Canine Eye Registration Foundation (CERF), respectively, as evidence of the defect. The guarantee consists of replacement of the dog with a dog of equal or better quality, selected by the BREEDER, when BREEDER has next available litter of puppies.

4. BREEDING CONTRACT (check if applicable)

BUYER agrees not to breed this animal until hips have been rated fair or better by OFA and eyes have been rated clear at a time no earlier than 24 months of age.

5. SPAY/NEUTER CONTRACT (check if applicable) *

BUYER agrees to have this animal spayed/neutered by the age of six months. At that time full registration will be possible. The entire agreement between the parties in contained herein.

BUYER has read, and understands, the above and agrees to the terms and has received a copy of this agreement.
