The Canadian Manchester Terrier Club is pleased to provide breeders with a new tool in the fight to safeguard the health of our breed.  The Manchester Terrier Check-Up was designed to provide a formal method of collecting data on the health of puppies you have placed in companion homes.

By monitoring the successes and challenges faced by companion owners, every Manchester breeder will be better able to modify their breeding strategies to produce happier, healthier puppies.

The Health Committee also asks that you store the information you collect so that is can be easily incorporated into your responses to the CMTC General Health Surveys.  This more accurate information will improve our ability to track trends and problems in the breed as a whole.

How to Check-Up

Print

The CMTC Health Committee recommends that the survey be distributed to each pet owner after 1 year, 5 years and 15 years.  To streamline the process you may wish to choose just one or two dates per year on which you will mail out survey forms annually.  It is further recommended that the identification portion of the form be filled out by you before mailing as this data may not be easily located by the pet owner as time passes.

A complete Print Package is available free of charge.

Electronic

Have a website and understand webforms?  You are welcome to copy the Breeder's Check-Up model and use it on your webpage free of charge ...OR... for only $25/year, the CMTC can set up and host a Check-Up Form for you!

How does it work?  Simply choose the questions you would like answers for, provide us with the email address you want information sent to and we'll do the rest.  The resulting page will automatically email data gathered directly to your Inbox (and only your Inbox).

In addition to choosing what information is gathered, you may also customize the thank you page in order to provide a special note to your buyers requesting photos, asking them to consider donating to health research or rescue, or perhaps providing your email address or website.

All funds raised will be used to support Manchester Terrier Health Projects.

How Often to Check-Up

We recommend that breeders maintain a list of email contact information for their puppy buyers.  Depending on your needs, you may choose to send a form reminder every year, every second year or perhaps on a schedule according to birth year (i.e., at 1 year, 5 years and 15 years).  Simply email the link to your clients and voila, you have a direct information conduit with minimal effort.  You do not need to have a website to make use of this system, though weblinks/graphics are available should you wish to link directly to your Check-Up form.

How to Pay

Payments can be made by cheque, money order or via Pay Pal.  Please email for more information.
